Membership Agreement, Consent and Disclosure Agreement

Consents and Agreements for Bad Wolf Chess Club LLC and Its Owners and Affiliates

• I understand that children under the age of 11 or as deemed necessary by Bad Wolf Chess Club LLC must have a parent or guardian present for all club meetings. The age limit for parental oversight at club meetings is subject to change at the discretion of Bad Wolf Chess Club LLC and its affiliates.


• All payments to Bad Wolf Chess Club are due by the first day of the session I or my child(ren) will attend a session, or if starting in the middle of a session, the prorated amount is due by the first meeting I or my child(ren) attends in the given session. Those who have not met this obligation will be asked to leave the current meeting and will not be allowed to join subsequent meetings until payment is made.


• I understand that if I should cancel my or my child(ren)’s membership, I will not be given a refund, prorated or otherwise, for the session I or my child withdrew.


• I understand that club meetings are not prorated. For example, if the member misses a club meeting or a club meeting was cancelled for reasons detailed below, I am still required to pay the full membership fee amount, except in cases of extreme circumstances, as determined by and at the discretion of Bad Wolf Chess Club LLC. There are no make-up sessions for missed club meetings; however, Bad Wolf Chess Club may offer a make-up session at their discretion of date and time. Cancelled club meetings due to inclement weather, force majeure events (e.g., but not limited to acts of war, acts of God, pandemic, government declarations), vacation and/or illness of the coach(es) of Bad Wolf Chess Club LLC, members will be notified as soon as possible by e-mail and text.


• I accept all injury or accident or illness that may occur in conjunction with this program to me or my child(ren). Nathan Easterday, Bad Wolf Chess Club LLC, Aquinas Academy and all of their affiliates provide no insurance for participants. I am releasing and hold harmless, Nathan Easterday, Bad Wolf Chess Club LLC, Aquinas Academy and all of their affiliates for claims, costs, and liabilities, and expenses including judgments, attorney’s fees and court costs. I understand that Nathan Easterday, Bad Wolf Chess Club LLC, Aquinas Academy and all of their affiliates are not responsible for any lost, stolen, or misplaced property or injury. I am executing this waiver in order for me and/or my child(ren) to participate in this program.


• I give consent to Nathan Easterday, Bad Wolf Chess Club LLC and all of their affiliates use of photography or video of me and/or my child(ren). I understand that photographs and videos may be used for advertising purposes, on the web site, Facebook, or other digital or print media. This consent will stay in effect unless rescinded in writing. Images or videos already in digital or print media prior to being rescinded will remain in use, including new uses in digital and print media, unless removed at the discretion of Bad Wolf Chess Club LLC or as ordered by a court of local jurisdiction.
